Patient was asymptomatic 10days ago then developed neck pain which is insidious in onset.Then she developed fever 2 days ago.Fever is intermittent mostly seen during night time a/w leg and hand pain which is dragging type and also sweating.No tenderness seen.On medication fever reduced for a while then again reoccured . She also has slight cold. Past history : Shortness of breath seen on walking few steps,No  h/o DM, No h/o HTN , No thyroid Past surgical history: undergone tubectomy 15 yrs ago Personal history : Appetite reduced Sleep not adequate Mixed diet Bowel bladder movements normal General examination:  Grade 1 edema No pallor  No icterus No cyanosis No clubbing No lymphadenopathy

Patient developed pain a/o swelling in right leg (on twisting his leg and falling down) which is insidious in onset.Pain is continuous, progressive.Restricted movement of legs.He also has pain in right hypochondrium. Past history : known case of HTN is metalol medication No h/o DM No h/o Asthma Past surgical history: endoscopic gall bladder stenting Personal history : Appetite normal, mixed diet, sleep adequate, bowel, bladder movements are difficult General examination : patient is examination in well ventilated room Patient is conscious, coherent, Moderately build, nourished Have pitting edema No pallor No icterus No cyanosis No clubbing No lymphadenopathy

Patient developed fever followed by pain in bilateral hypochondrium and chest 5 days ago,which is insidious in onset.Continuous, radiating type of pain.No tenderness seen. Aggrevated on lying down, on cough.a/w cold. Past history : H/o giddiness, headache Past surgical history : tubal (ectopic)pregnancy surgery Personal history :  mixed appetite, appetite sufficient, sleep adequate , bowel bladder movements are normal Obs H/o : G2P2L2A1, c-section General examination : Moderately build, conscious,coherent,no pallor ,no icterus, no clubbing,no cyanosis, no edema, no lymphadenopathy
